It is recommended that all candidates for the CDMF submit an experience verification form at the time of enrollment. the experience verification form must be recieved by AMSC prior to obtaining your CDFM.

CDFM Program Enrollment Form

To apply and sit for the examination a candidate must:

• Promise to adhere to the ASMC Pledge of Professionalism and
• Have a high school diploma or equivalent

You can either Enroll Online, or mail your payment with the enrollment fee (check, American Express, Visa or MasterCard), to the address shown on the application.
